Self-referral

If you wish to refer yourself for mental health support from the Health for Health Professionals Wales service please complete the form below.

Due to the nature of our service we are not able to accept referrals made on behalf of someone else.

Referral
We will arrange a call for you with one of our doctor advisors. Please can you let us know your availability Monday – Friday between 9am and 5pm. Byddwn yn trefnu galwad i chi gydag un o’n cynghorwyr. A allwch roi gwybod i ni pan fyddwch ar gael o ddydd Llun i ddydd Gwener rhwng 9yb a 5yp.

​Once the HHP Wales team have received your referral form, an appointment will be made for you to have a telephone call with one of our Dr Advisors. 

HHP Wales Dr Advisors work from 9am-5pm Monday to Friday (excluding Bank Holidays).

You will be emailed, within one working day of receipt of your form, with confirmation of the details you have provided in the referral form along with the date and time of your telephone appointment with a Dr Advisor.

The Workforce Specialist Service (WSS), delivered by NHS Practitioner Health, is a confidential multidisciplinary mental health service with expertise in treating regulated health and social services professionals. They specialise in caring for regulated professionals as patients, and as such are experts at the interface between regulation, employment and mental illness and addiction.

Accessing support in England

Practitioner Health is a free, confidential NHS service for doctors and dentists across England with mental illness and addiction problems, who are working or looking to return to clinical practice.

The service can help with issues relating to a mental health concern, including stress or depression or an addiction problem, in particular where these might affect work. The service is provided by health professionals specialising in mental health support to doctors and is available in various locations across England
